• Provide professional development and consultative services to develop strategies to bridge school programs and the opportunities of adult life, including higher education, employment, independent living and community participation.

• Support school staff to build partnerships between students, families, school-age services/program providers, post-school services/program providers, and local community members

• Enhance staff proficiency in assessment of student strengths, interests, preferences, and needs, and build knowledge of services and supports needed to achieve future success.
